The Piedmont Wildlife Center Records include medical records, 2003-2009, of Piedmont Wildlife Center, including intake forms and logs, physical exam forms, lab logs, and a record of data from the North Carolina Bioterrorism and Emerging Infection Prevention System (BEIPS). Records include information on species of animal admitted, its injury, found orphaned or ill, location where found, person bringing animal to the hospital and subsequent information about examination findings, treatment, or disposition (released, died in care, euthanized, other).

Scope/content

The Piedmont Wildlife Center Records include medical records, 2003-2009, of Piedmont Wildlife Center, including intake forms and logs, physical exam forms, lab logs, and a record of data from the North Carolina Bioterrorism and Emerging Infection Prevention System (BEIPS). NC BEIPS has received electronic data from PWC since January 1, 2005.

Records include information on species of animal admitted, its injury, found orphaned or ill, location where found, person bringing animal to the hospital and subsequent information about examination findings, treatment, or disposition (released, died in care, euthanized, other).

Nearly all items are compiled into binders, most of which have a front cover describing what is included inside and the year. They appear to be arranged in chronological order, at least by year if not also by specific date and form number. Some items are in composition notebooks, like lab notes, but those comprise a small percentaage of the collection compared to material in binders.

Arrangement

This collection has not received full archival processing. Most materials remain in binders and the binders have been grouped by the type of records in them.

This collection may contain materials with sensitive or confidential information that is protected under federal or state right to privacy laws and regulations. Researchers are advised that the disclosure of certain information pertaining to identifiable living individuals represented in this collection without the consent of those individuals may have legal ramifications (e.g., a cause of action under common law for invasion of privacy may arise if facts concerning an individual's private life are published that would be deemed highly offensive to a reasonable person) for which North Carolina State University assumes no responsibility.
