3. If your form is submitted successfully, you will be forwarded to a confirmation page.

By Fax

1. Check our online catalog to see if we own the document you'd like to request.
2. Complete Document Supply Services Sheet.
3. Fill out citation information on the second sheet.
4. Fax both sheets back to the Textiles Library at (919) 515-3926.
